Confirming your coverage

Insurance coverage, copays, deductibles, and prior-authorization requirements vary by plan and policy. Before your appointment, please call our office at (907) 267-5890 so we can verify whether your specific plan is in-network and what to expect at your visit.

What to bring to your visit

  • A photo ID (driver's license, state ID, or passport)
  • Your current insurance card(s), including any secondary or supplemental coverage
  • Any referral or prior-authorization paperwork your plan requires
  • A list of current medications, including over-the-counter products and supplements
  • Payment method for any copay, coinsurance, or deductible owed at the time of service

Referrals & prior authorizations

Some health plans (including certain HMO, Medicare Advantage, Tricare, and Medicaid plans) require a referral from your primary care provider before they will cover a dermatology visit. Other plans may require prior authorization for specific procedures, biologic medications, or imaging. We recommend checking your benefits with the member-services number on the back of your insurance card before scheduling.

Self-pay & cosmetic visits

Cosmetic services and certain elective procedures are typically not covered by insurance. We are happy to provide a written estimate of charges in advance for self-pay and cosmetic visits. Please ask our front-desk team when you schedule.

Billing, statements & payments

After your visit, your insurance will be billed first. Any remaining balance owed by you will appear on a statement. You can pay your bill online through our secure payment portal:

Good Faith Estimate (uninsured & self-pay)

Under the federal No Surprises Act, patients who are uninsured or who choose not to use their insurance have the right to receive a "Good Faith Estimate" for the cost of non-emergency items and services. Please contact our billing team at (907) 267-5890 to request an estimate before your visit.
